Homemaker

 We are looking for a Homemaker and caregiver to provide in-home care to our clients. Your primary function is to ensure daily tasks are completed to create a safe, clean, and welcoming environment. You will assist with grocery shopping and meal preparation, so our ideal candidate is someone with cooking skills and is comfortable cooking for people with some dietary restrictions. You will also perform various cleaning tasks, including sweeping, vacuuming, dusting, and washing dishes. In addition to your cooking and cleaning tasks, you will provide companionship to elderly members of the household, transporting them to social activities and ensuring they remain both mentally and physically engaged. 


Homemaker Duties and Responsibilities

  • Prepare meals
  • Shop for groceries
  • Wash dishes
  • Do laundry and change bed linens
  • Perform any additional household cleaning tasks, including vacuuming and dusting
  • Provide companionship
  • High school diploma or GED certificate preferred
  • Experience cooking for people with dietary restrictions a plus
  • Familiarity with common household cleaning products and techniques
  • Reliable form of transportation
  • Communication skills

Personal Care Assistant (PCA)

We are currently seeking a compassionate, highly-skilled, and reliable personal care assistant (PCA) to serve as part of our growing A-team. In this role, you will work with our clients in their homes, helping them with basic personal hygiene and domestic tasks as needed, according to their requirements and limitations. You will provide compassionate care and assistance and document clients’ progress to help ensure their overall wellbeing.


PCA Duties and Responsibilities

  • Travel to clients’ homes, arriving on time
  • Greet clients in a warm, professional manner
  • Understand the limitations and needs of each individual client
  • Provide basic domestic services meal prep, laundry, light housework
  • Provide companionship and engage in conversation with the clients
  • Run errands, either with or for the client, as needed
  • Assist with mobility issues, helping the client get in and out of bed, wheelchair, etc.
  • Remind the client to take medications and document them accordingly
  • Help with personal hygiene e.g. brushing teeth, toileting, bathing
  • Change bed linens regularly
  • Assist with all daily activities, based on the client’s limitations and needs
  • Provide a caring and positive presence for the client
  • Document/log client’s’ progress and overall well being for daily reports 


PCA Requirements and Qualifications

  • High school diploma or equivalent required
  • Current certificate in first aid and CPR preferred
  • Valid driver’s license, reliable automobile, and current auto insurance required
  • Minimum one year of experience working in a health care setting
  • Significant experience working in a home health care environment is strongly preferred
  • Experience in domestic work or housekeeping is a plus
  • Mastery of the English language, written and verbal
  • Good communication skills, excellent conversational skills
  • Able to lift heavy objects, help move clients, and perform physical tasks
  • Patience and ability to remain calm in stressful situations
  • Willingness to conduct domestic and personal tasks requested by the client
  • Prompt and reliable
  • Warm and caring personality
  • Commitment to treating clients with dignity and respect
  • Willingness to comply with all Minnesota legal and institutional requirements and guidelines 

Independent Living Services (ILS)

 We are currently looking for an Independent Living Specialist who has a strong voice for disability rights and independent living, working to assist clients in maintaining their lives independently in the community. Promotes clients and collaborates with community agencies to best assist the client to reach goals for independent living. The Independent Living Specialist will provide general information and referral for consumers and the community as requested. The Independent Living Specialist works as a team player to achieve the stated goals and objectives of all programs that are accessible. 


Duties and Responsibilities

  • Prepare teaching material for Independent Living skills Training and Support Service
    sessions.
  • Schedule client session time and programming according to their Individual Service Plan
    and the training domains.
  • See individual clients in their home to train, teach and provide support in: money management, nutrition, cooking, comparison shopping, emergency procedures, community awareness, mobility, laundry procedures, household management, sex/public health education, medical procedures, counseling and crisis intervention, and housing
  • Provide training in rations determined by current vendorization, and in accordance with the least restrictive environment philosophy (1:1 staff: client ration, unless otherwise specified).
  • Coordinate client training programs with other programs with which they are involved.
  • Write Individual Service Plans as required by current regulation. Write semi-annual
    Progress Reports based on ISP objectives.
  • Complete monthly Program Hours sheets documenting direct hours spent with clients
    and submit to supervisor(s) on the first day of the following month.
  • Report suspected abuse as immediately to supervisor, case manager and other
    appropriate agency as instructed. Complete special incident report within 24 hours.
  • Keep client files up to date.
  • Assist client in coordinating with other programs, services and benefit systems, such as
    Social Security, AFDC, WIC, food stamps, Planned Parenthood, etc.
  • Attend monthly staff meetings, and monthly individual meetings with supervisor.
